3. The administrator will process the following personal data: name and surname, name of the represented or employing entity, position, contact telephone number, e-mail address.
4. The data provided will be processed pursuant to Art. 6 section 1 point a) and point b) of the General Data Protection Regulation of April 27, 2016
5. Your personal data will be processed on behalf of the data controller by authorized employees only for the purposes referred to in paragraph 4
6. The recipients of your personal data will be entities providing services to the administrator and the “THINK-TANK. PREVENTION & INTERVENTION” FOUNDATION, Marka Hłaski 26 Street, 44-100 Gliwice, KRS 0001094239, TAX PL6312720799, REGON 528064959
7. Providing data is voluntary but necessary in order to provide our services

The provided personal data will be stored during the period of cooperation and after its termination for a period of 5 years for statistical and archiving purposes, in order to fulfill accounting obligations related to separate legal provisions, as well as to pursue potential claims related to the provision of the service
9. You have the right to access your data and rectify it, delete it, limit processing, the right to transfer data, the right to withdraw consent at any time without affecting the lawfulness of processing
10. You have the right to lodge a complaint with GIODO (Stawki 2 Street, 00-193 Warsaw) if you believe that the processing of your personal data violates the provisions of the General Data Protection Regulation of 27 April 2016;”
